SHIP 4.0 and Healthcare

This iteration of SHIP has three healthcare components:

  • Worksite Wellness

  • Breastfeeding Coalition

  • Omaha System Documentation

Worksite Wellness

This component is public health partnering with healthcare settings to encourage participation in the PartnerSHIP 4 Health Worksite Wellness Collaborative. We are currently seeking partners for the Fall 2016 Collaborative.

Breastfeeding Coalition

This component is public health partnering with healthcare to establish a local breastfeeding coalition called Partnership4Breastfeeding.

Omaha System Documentation (Approved SHIP Variance)

This component is public health working with the Omaha System Community and Key Stakeholders to develop and pilot an Omaha System Pathway for practice documentation and outcomes evaluation related to SHIP strategies.

Community Wellness Grant and Healthcare

The Community Wellness Grant (CWG) has three components related to healthcare:

  • Health System Intervention

  • Community Clinical Linkages

  • Lifestyle Change

Health System Intervention

This component is public health partnering with healthcare settings to address prediabetes and hypertension among their patient population by putting scientific-based guidelines into practice. Health care partners are empowered to integrate the steps of “Ask, Advise, Assess Readiness, Assist, and Arrange Follow-up” related to prediabetes and hypertension into clinic and practice flow.

In addition, because a “culture of wellness” enhances clinician ability to have wellness discussions with their patients, PartnerSHIP 4 Health works with healthcare partners to advance worksite wellness in their respective settings by encouraging participation in the PartnerSHIP 4 Health Worksite Wellness Collaborative.

This component is public health partnering with Community Paramedics and Community Pharmacists to engage in hypertension management, and to develop a bi-directional referral system related to the I CAN Prevent Diabetes Program.

Lifestyle Change
